Submitting a Spa Services request form does not guarantee appointment. All appointments must be confirmed by submitting a credit card number. Upon receipt of this form our spa associates will review your request and contact you to confirm the date/time and service and to obtain the necessary credit card information. All appointments are subject to a 24-hour cancellation policy except for our Treatment Packages which require 72-hours' cancellation notice as noted below. Any service cancelled or rescheduled with less than a 24-hour notice (or 72-hour notice as in the case of Spa Treatment Packages) will be subject to 50% charge. By providing your credit card number you agree to all of our policies (if you have not read them yet, use select "Booking and Cancellation Policies" above, left).
